Key Responsibilities
- Serve as the primary point of contact for patients and their families, guiding them through their healthcare journey.
- Provide comprehensive education on diagnoses, treatment plans, medication management, and self-care techniques in an understandable manner.
- Assess patient needs for various resources, including financial assistance, transportation, social support, and mental health services, and facilitate appropriate referrals.
- Coordinate appointments, tests, and follow-up care, ensuring adherence to treatment protocols.
- Advocate for patient rights and preferences, ensuring their voices are heard and respected in care planning.
- Maintain accurate and confidential patient records, documenting all interactions and care coordination efforts.
- Collaborate effectively with physicians, nurses, social workers, and other healthcare professionals to ensure integrated and holistic patient care.

Required Skills
