Key Responsibilities

  • Conduct comprehensive patient assessments to identify care needs, barriers, and potential risks.
  • Develop, implement, and monitor individualized care plans in collaboration with patients, families, and the interdisciplinary healthcare team.
  • Coordinate appointments, referrals, and necessary medical procedures, ensuring timely access to services.
  • Educate patients and families on their health conditions, treatment options, self-management techniques, and available community resources.
  • Facilitate effective communication among all members of the healthcare team to ensure coordinated and holistic care.
  • Assist with discharge planning, connecting patients with post-acute care services, home health, and durable medical equipment as needed.
  • Advocate for patient rights and preferences, ensuring patient-centered care decisions.
  • Document all patient interactions and interventions accurately and thoroughly in the electronic health record (EHR).
  • Identify and address social determinants of health that impact patient well-being and access to care.
  • Participate in quality improvement initiatives and team meetings to enhance care coordination processes.

Required Skills

  • Current RN license in Arkansas (or ability to obtain before start date)
  • Minimum of 3 years of clinical experience in an acute care setting or care management role
  • Strong understanding of healthcare systems, insurance, and community resources
  • Excellent verbal and written communication skills
  • Demonstrated ability to perform critical thinking and problem-solving in complex situations
  • Proficiency in electronic health record (EHR) systems
