Key Responsibilities
- Conduct comprehensive patient assessments to identify care needs, barriers, and potential risks.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and the interdisciplinary healthcare team.
- Coordinate appointments, referrals, and necessary medical procedures, ensuring timely access to services.
- Educate patients and families on their health conditions, treatment options, self-management techniques, and available community resources.
- Facilitate effective communication among all members of the healthcare team to ensure coordinated and holistic care.
- Assist with discharge planning, connecting patients with post-acute care services, home health, and durable medical equipment as needed.
- Advocate for patient rights and preferences, ensuring patient-centered care decisions.
- Document all patient interactions and interventions accurately and thoroughly in the electronic health record (EHR).
- Identify and address social determinants of health that impact patient well-being and access to care.
- Participate in quality improvement initiatives and team meetings to enhance care coordination processes.
Required Skills
- Current RN license in Arkansas (or ability to obtain before start date)
- Minimum of 3 years of clinical experience in an acute care setting or care management role
- Strong understanding of healthcare systems, insurance, and community resources
- Excellent verbal and written communication skills
- Demonstrated ability to perform critical thinking and problem-solving in complex situations
- Proficiency in electronic health record (EHR) systems
- Exceptional organizational and time management abilities
- Ability to work effectively in a fast-paced, team-oriented environment

Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
