Job Description

CISSS de Lanaudière is actively seeking a motivated Community Support Worker to join our dynamic team in Terrebonne, Quebec. As a Community Support Worker, you will work directly with clients facing diverse circumstances, including mental health issues, social integration difficulties, elderly care, or individuals requiring support due to physical or intellectual disabilities. Your day-to-day will involve a mix of direct client interaction, case management, and collaborative work with interdisciplinary teams. You will be instrumental in developing and implementing personalized intervention plans, connecting clients with vital community resources, and fostering their strengths and abilities.

Key Responsibilities
- Assess client needs and develop individualized intervention plans in collaboration with the client and interdisciplinary team.
- Provide direct support, guidance, and accompaniment to clients in various settings (home, community, institutions).
- Assist clients in developing life skills, managing daily routines, and achieving personal goals.
- Facilitate access to community resources, including housing, employment, education, and healthcare services.
- Advocate on behalf of clients to ensure their rights and needs are met.
- Monitor client progress, document observations, and adjust intervention plans as necessary.
- Collaborate effectively with social workers, nurses, psychologists, and other professionals.
- Participate in case conferences, team meetings, and professional development activities.
- Maintain accurate and confidential client records in accordance with organizational policies and ethical guidelines.
- Respond to crisis situations with sensitivity and professionalism, implementing appropriate intervention strategies.
Required Skills
- Diploma of Collegial Studies (DEC) in Social Work, Human Services, Special Education, or a related field
- Minimum of 1 year of experience in a similar community support or social service role
- Strong understanding of the challenges faced by vulnerable populations
- Excellent interpersonal, communication, and active listening skills
- Ability to work autonomously and as part of a multidisciplinary team
- Demonstrated empathy, patience, and non-judgmental approach
- Proficiency in French (oral and written)
- Valid driver's license and access to a reliable vehicle
Preferred Qualifications
- Bachelor's degree in Social Work or a related field
- Bilingualism (French and English)
- Experience working with mental health clientele
- Knowledge of local community resources in Terrebonne and the Lanaudière region
- Crisis intervention training
