Job Description
Are you a compassionate and driven individual with a passion for community well-being? Hopeville Foundation is seeking a dedicated Community Health Worker (CHW) to join our public health and outreach team in Rumuokwuta, Port Harcourt. In this crucial role, you will serve as a vital link between our programs and the local community, bridging gaps in healthcare access and information. You will engage directly with residents, providing essential health education, promoting preventive care, and connecting individuals and families with available health services and resources. This position requires someone who is culturally sensitive, an excellent communicator, and deeply committed to empowering individuals to take charge of their health. You will be instrumental in identifying community health needs, advocating for residents, and supporting the implementation of health interventions designed to improve public health outcomes in the Rumuokwuta area. Key Responsibilities
- Conduct community outreach activities, home visits, and group education sessions to engage residents on critical health topics.
- Provide accurate and culturally appropriate health information and education on maternal and child health, nutrition, hygiene, communicable diseases (e.g., malaria, HIV/AIDS), non-communicable diseases, and family planning.
- Facilitate access to healthcare services by assisting community members with referrals, appointments, and navigation of the health system.
- Act as an advocate for community members, communicating their needs and concerns to healthcare providers and program staff.
- Collect and document health data, conduct basic health screenings (e.g., blood pressure, weight), and maintain accurate records of client interactions and program activities.
- Organize and support community health events, workshops, and awareness campaigns.
- Build and maintain strong, trusting relationships with community leaders, local authorities, and residents.
- Identify social determinants of health impacting the community and work towards addressing them through appropriate referrals and interventions.
- Collaborate effectively with other CHWs, nurses, doctors, and project staff to ensure coordinated care and comprehensive service delivery.
- Participate in regular team meetings, training sessions, and supervision to enhance skills and knowledge.
Required Skills
- Proven experience in community engagement, outreach, or health promotion roles.
- Excellent verbal and written communication skills in English and proficiency in local languages (e.g., Ikwerre, Pidgin English).
- Strong interpersonal skills with the ability to build rapport and trust with diverse populations.
- Demonstrated cultural sensitivity and empathy towards individuals from various backgrounds.
- Ability to work independently with minimal supervision and as part of a collaborative team.
- Basic computer literacy for data entry and record keeping.
- Organizational skills and attention to detail for accurate documentation.
- Problem-solving abilities and a proactive approach to community challenges.

Preferred Qualifications
- Certificate or diploma in Community Health, Public Health, Social Work, or a related field.
- Previous experience as a certified Community Health Worker (CHW) or Health Volunteer.
- Knowledge of local health challenges and community resources in Rivers State, particularly Rumuokwuta.
- Experience with mobile data collection tools or basic survey administration.
- Valid driver's license or ability to commute reliably within the community.
