As a valued member of our Case Management team, you will be instrumental in conducting comprehensive psychosocial assessments, identifying diverse patient needs, and developing meticulously individualized care plans tailored to each patient’s unique circumstances and goals. Your day-to-day responsibilities will encompass coordinating complex discharges, meticulously arranging for a wide array of post-acute care services such as home health, rehabilitation, skilled nursing facilities, and palliative care. Furthermore, you will expertly connect patients with vital financial assistance programs, housing support, mental health services, and various community support groups, addressing barriers to recovery comprehensively. You will play a pivotal role in preventing avoidable readmissions by proactively identifying and addressing social barriers to recovery, educating patients on self-management strategies, and fostering independence. Key Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized care plans.
- Provide crisis intervention, emotional support, and counseling to patients and families facing health challenges.
- Collaborate with interdisciplinary healthcare teams to ensure holistic, patient-centered care and optimal outcomes.
- Coordinate complex hospital discharges, arranging for post-acute care services and necessary community resources.
- Advocate for patients' rights and preferences, ensuring ethical practice and patient empowerment.
- Connect patients with financial assistance, housing support, mental health, and other community-based services.
- Educate patients and families on disease management, prevention strategies, and self-care techniques to reduce readmissions.
Required Skills
- Master's Degree in Social Work (MSW) from an accredited program
- Current New Mexico Social Work License (LCSW or LMSW eligible)
- Minimum of 2 years of experience in case management, social work, or a related field within a healthcare setting
- Strong understanding of psychosocial factors impacting health and recovery
- Exceptional communication, interpersonal, and advocacy skills
- Proficiency in electronic health records (EHR) systems and Microsoft Office Suite
- Ability to work effectively in a fast-paced, dynamic hospital environment
Preferred Qualifications
- Experience in an acute care hospital setting
- Bilingual (English/Spanish) communication skills
- Certification in Case Management (CCM)
