As a Nurse Practitioner with Lifemark, you will play a pivotal role in providing comprehensive, patient-centred primary care and chronic disease management. Key Responsibilities

  • Conduct comprehensive health assessments, including medical history, physical examinations, and diagnostic tests.
  • Diagnose and manage acute and chronic illnesses, developing individualized treatment plans in collaboration with patients.
  • Prescribe medications and other therapies in accordance with provincial regulations and scope of practice.
  • Order, interpret, and evaluate diagnostic and laboratory tests.
  • Provide health education, counseling, and preventative care services to diverse patient populations.
  • Collaborate effectively with physicians, registered nurses, and other allied health professionals to ensure integrated patient care.
  • Maintain accurate, confidential, and detailed electronic medical records (EMRs).
  • Participate in quality improvement initiatives, team meetings, and continuing professional development activities.
  • Act as a patient advocate, ensuring patient-centered care and shared decision-making.
  • Refer patients to specialists or other healthcare providers as appropriate.

Required Skills

  • Master's degree in Nursing (MN) or Doctor of Nursing Practice (DNP) with a Nurse Practitioner specialty from an accredited institution.
  • Current registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class (Nurse Practitioner).
  • Minimum 3 years of clinical experience as a Nurse Practitioner in a primary care or relevant specialty setting.
  • Demonstrated knowledge of advanced nursing practice, pharmacology, and health assessment.
  • Strong communication, interpersonal, and critical thinking skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Proficiency in electronic medical record (EMR) systems.
  • Eligibility for sponsorship and commitment to navigating the immigration and licensure process for international candidates.

Preferred Qualifications

  • Experience with a diverse patient population, including newcomers to Canada.
  • Additional certifications in specialty areas (e.g., wound care, gerontology, mental health).
  • Bilingualism (e.g., English and Mandarin, Cantonese, Hindi, or other languages prevalent in Markham).
  • Previous experience in a private clinic or multidisciplinary rehabilitation setting.
  • Demonstrated leadership potential and involvement in professional organizations.
