Key Responsibilities
- Conduct comprehensive, holistic, and culturally sensitive assessments of members’ health status, needs, and preferences, utilizing advanced clinical expertise and established protocols.
- Develop, implement, and rigorously monitor individualized, evidence-based care plans in close collaboration with members, their families, and a diverse network of healthcare providers, addressing complex medical, social, and emotional needs.
- Strategically coordinate and facilitate timely access to a wide range of essential healthcare services, including specialist consultations, intricate home health services, durable medical equipment, and vital community resources.
- Provide extensive education and empathetic support to members and their families on complex health conditions, medication management, effective self-care techniques, and available benefit options.
- Act as a primary point of contact and an unwavering advocate for members, ensuring their voices are authentically heard and their needs are comprehensively met throughout their entire care journey.
- Proactively identify and analyze potential barriers to care, developing innovative strategies to overcome them, thereby ensuring continuity and maximal effectiveness of services.
- Facilitate seamless and safe transitions of care between diverse healthcare settings (e.g., hospital to home, skilled nursing facility), actively minimizing readmissions and adverse events.
- Maintain exceptionally accurate, timely, and thorough documentation of all interactions, assessments, and care plan activities in strict compliance with regulatory and company standards.
- Actively participate in multidisciplinary team meetings and collaborate effectively with physicians, nurses, social workers, and other professionals to continuously optimize member outcomes.
- Adhere strictly to all ethical, legal, and professional standards of practice, meticulously maintaining confidentiality and upholding member rights.
- Proficiently utilize technology and digital tools for efficient communication, comprehensive documentation, and streamlined care coordination.
- Continuously evaluate the effectiveness and impact of care plans, adjusting them as necessary based on member progress, evolving needs, and emergent clinical data.
Required Skills
- Registered Nurse (RN) with current valid NMC registration (or equivalent UK healthcare professional qualification in a related field).
- Minimum of 3 years of post-qualification clinical experience in an acute care, community health, or managed care setting, demonstrating progressive responsibility.
- Proven extensive experience in case management, complex care coordination, or sophisticated discharge planning.
- Exceptional assessment, critical thinking, and advanced problem-solving skills with the ability to navigate ambiguity.
- Superior interpersonal, communication (both verbal and written), and negotiation skills, capable of influencing positive outcomes.
- Demonstrated ability to work autonomously, exercising sound judgment, and managing a diverse and complex caseload effectively.
- Proficiency in using electronic health records (EHR) systems and standard office software (e.g., Microsoft Office Suite, virtual collaboration tools).

Preferred Qualifications
