Job Description

Key Responsibilities

  • Provide direct support and assistance to clients in various community and home settings, promoting their independence and quality of life.
  • Develop and implement personalized intervention plans in collaboration with clients, their families, and the multidisciplinary team.
  • Assist clients with daily living activities, including personal care, household management, meal preparation, and medication management.
  • Facilitate access to community resources, such as housing, employment, education, and recreational activities.
  • Support clients in developing essential life skills, coping strategies, and social competencies.
  • Monitor client progress, document observations, and report any significant changes or concerns to the appropriate team members.
  • Act as a liaison between clients, their families, and other service providers, ensuring coordinated and holistic care.
  • Participate in case conferences and team meetings to review client needs and adjust support plans as required.
  • Adhere to all organizational policies, procedures, and ethical guidelines, ensuring client confidentiality and safety.
  • Engage in continuous professional development to enhance skills and knowledge related to community support work.

Required Skills

  • Diploma of College Studies (DEC) in Special Education, Social Work, or a related field, or equivalent experience.
  • Minimum of 1 year of experience working in a community support, social services, or healthcare setting.
  • Excellent communication and interpersonal skills, with a strong ability to build rapport and trust.
  • Demonstrated empathy, patience, and a non-judgmental approach to client care.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organizational and problem-solving abilities.
  • Proficiency in both French and English (oral and written) is highly desirable.
  • Valid driver's license and access to a reliable vehicle for community visits.

Preferred Qualifications

  • Experience with mental health first aid or crisis intervention training.
  • Knowledge of local community resources and support networks in the Outaouais region.
  • Previous experience working with diverse populations or individuals with specific needs (e.g., mental health, developmental disabilities, seniors).
  • Certification in CPR and First Aid.
